Can a policy be revised without re-acknowledgement?

Yes. Updating the body of an existing version is allowed for typos. Publishing a new version (v2 → v3) is the trigger for re-acknowledgement: each version has its own ack table row per employee.

Where does the document expiry alert go?

Wherever a notification template is configured for DOCUMENT_EXPIRY_ALERT — typically the employee's email + in-app, plus an HR digest. The expiry watch carries an owner_employee_id so the right person is paged.

Code + version + body in HTML, with branding placeholders that pull tenant logo and footer from tenant config. Re-acknowledgement cadence per policy (e.g. annual for CoC).

Acknowledgement = signature + IP

When an employee acknowledges, we capture acknowledged_at, optional typed signature, and the IP address as audit evidence. Updates overwrite the existing row, not create new ones, so the audit query stays simple.

Document expiry never slips

Daily 07:00 cron finds documents within their configured lead-days of expiry and emits DOCUMENT_EXPIRY_ALERT events. Notification templates handle the email + in-app render.
